Schedule changes will be granted by administration only if the need is warranted. Check the option that applies to you and complete the form with the appropriate signatures. Students MUST follow their current schedule until the new schedule is received. Please note that making schedule changes may result in unexpected additional changes to students' schedules in order to maintain balanced classes so be very careful about what you are asking for.

NOTE: All final decisions will be communicated by email and/or text using the email addresses/phone numbers provided at the bottom on this form.

Also, you may not request to change teachers, periods, or lunch.) Option 1: (11th/12th grade only) The schedule is missing a Graduation Requirement. Option 2: The student has already earned credit for this course and needs a different course in its place. Option 3: The student is going to take a Pasco eSchool/FLVS/PHSC class in place of a ZHS class. (The student should have already signed up for the online class via eschool.pasco.k12.fl.us or flvs.net to avoid a delay in a schedule change. Dual Enrollment students should have already signed up for their DE classes with PHSC and can provide proof.) Option 4: There is at least one missing period in the schedule OR there are too many classes in the same period.
